Description of Services: Hospital transitional unit for patients who have a set discharge destination but have a temporary barrier such as waiting for home or community care services or convalescent care * Short term supports for quick and safe discharge, discharge plan must be in place before transferred to this unit * No direct admissions from emergency department * 40 beds
