Program: Transitional Care Inpatient Unit (TCU)

Description of Services: Hospital transitional unit provides services to current RVH patients and does not take outside referrals * 40-bed unit serves as a short stay for patients who are waiting for community support such as inpatient rehabilitation, Ontario Health atHome, RVH at home or private pay services * discharge plan must be in place prior to transfer to TCU with no direct admissions from the emergency department * patients are expected to participate in their plan of care and rehabilitation

  • occupational therapists
  • a discharge planner
  • social worker
  • a respiratory therapist

Eligibility - Population(s) Served: Patients discharged from hospital and have a discharge destination
Application: Referrals accepted from hospital staff * RVH internal referrals only
