Program: Diabetes Management Centre - Diabetes Education and Management

Description of Services: Patient-centred education and support to help people living with Diabetes or Prediabetes to manage their disease

Services include:
  • one-on-one sessions with registered dietitians and registered nurses (certified diabetes educators)
  • ongoing information and education 
  • insulin pump starts and support
  • endocrinology clinics (referral to the Endocrinologist is required)
  • group programming which includes activities and cooking

Requirements
Fees: Free
Eligibility - Population(s) Served: Adults 18 years and older with Type 1, Type 2 or Gestational Diabetes as well as those with Prediabetes
Application: Referrals from Physicians, Nurse Practitioners, or self-referrals are accepted
