Services available:
  • interdisciplinary team that consists of two social workers, an occupational therapist, a registered nurse, and a physician medical advisor
  • option to be seen in personal home, retirement residence, or hospital
  • works collaboratively with other community partners
  • support discharge for complex alternative level of care patients

Application: Referral from physicians or nurse practitioners
